- Please save the form to your computer. Open form, agree to terms and conditions by selecting the checkbox and signing your name and alias then submit form.
- Or, email a screen shot of the bottom portion of the Terms and Conditions form to [e-mail address] (be sure to include the selected checkbox and completed signature line). [Company program] will manually add you to the [infuriating] system.
